Social determinants of health definitions
Duties in the portal are tagged against recognised social determinants of health to show where they influence the wider conditions that shape people’s health and wellbeing. This helps users see connections beyond individual services or policy areas. The definition of each social determinant of health is set out below.

Looks at whether people have enough income and financial security to meet basic needs such as food, housing, heating and transport, which all strongly influence health and wellbeing. -
Health and social care
Refers to people’s ability to access timely, high-quality health services, social care, and support that help them stay well, manage long-term conditions, and live independently. -
Neighbourhood and environment
Covers the places where people live, work and travel, including housing quality, green spaces, safety, transport, and exposure to pollution, all of which shape daily health outcomes. -
Social and community context
Reflects the strength of people’s relationships, social networks, and sense of belonging, including support from family, friends and communities, and experiences of inclusion or discrimination. -
Children, families and lifelong learning
Focuses on early years, education, skills and learning across life, recognising that strong starts for children and ongoing learning opportunities support better health throughout adulthood.
