Simple terms explainer
Councils must work with NHS boards through IJBs to plan and deliver joined-up health and social care services. They look at what people in the community need - now and in the future - and design services to meet those needs using shared budgets. This involves developing a Strategic Plan, which sets out how services will be delivered to improve health and wellbeing, with involvement of service users, carers, professionals, and other partners to make sure services are person-centred, effective, and sustainable.
