This service was launched in Coatbridge Locality in January 2015. It is designed to provide a responsive, personalised and flexible service to people who require a high level of care. This may be as a result of discharge from hospital following a medical emergency or more commonly because of a need for end of life care.

The service works closely with carers and colleagues from Macmillan Cancer Care and Lanarkshire NHS.
Benefits and Impact
Of the 82 people who have received support, since January 2015, 72 have received palliative care. No one has had to be admitted to residential care and the cycle of reviews which we have held indicate that there is a 100 % satisfaction rate. Our service supports people to die with dignity, in their own homes, supported by family and friends.
Learning
Prior to the creation of the service people with end of life care needs often experienced frequent and largely unnecessary hospital admissions. This was, in part, caused by the fact that council and NHS operational arrangements were not jointly developed. With the introduction of this new service this has now radically changed and will continue to improve under the partnership arrangements.
